Job Description
Providing Intensive Outpatient, Standard Outpatient groups and Individual counseling for adults suffering with addiction.
Client assessments, evaluations, and appropriate referrals for discharge planning.
Essential Duties and Responsibilities
Conduct confidential, quality assessments, utilizing ASAM Placement criteria and the Addiction Severity Index to determine the appropriate level of care.
Use a strength-based, trauma-informed and person-centered approach collaborating with clients and internal and external team members to help clients to achieve their treatment goals.
While helping clients engage in the decision-making process, help clients complete a Treatment Plan that reflects what they wish to accomplish in treatment.
Be read into client’s charts, reviewing referral information and referral sources consulting with others as needed for coordination of discharge services.
Facilitate IOP and SOP groups Monday through Thursdays as scheduled. Flexible schedule with virtual groups allowed.
Remain in compliance with internal and external policies and procedures.
Participate in all assigned meetings, staff development, fundraising events, and training as required.
Requirements
Active MSW, LMSW, CAC-AC, CSC-AD, ADT with a clinical background. Also Peer Support Specialist.
Combination of license, comparable kills, experience and education will be considered.
Strong organizational and time-management skills.
Excellent written and verbal communication skills.
Job Type: Part-Time and Full-Time positions open. Flexible, day, afternoon and evening shifts available. Excellent Pay.
Company paid Sick Days, Vacation, Supplemental Health Plan, and retirement plan that Sober Truth matches from 1-3% annually. Benefits begin after a six-month probationary period. The Simple IRA Retirement plan can be added after one year of service.
