Job Description

Responsibilities:

Provide and/or coordinate the assessment of client needs, development of individual service plans (as assigned), linkages to internal and external services and resources and natural support systems, and advocacy.
Facilitate effective communication among various program components and with external service providers and natural support systems.
Address client concerns as they arise, i.e. resource coordination, appointments, transportation, housing, entitlements coordination, debt, money management, etc.
Transport and accompany individuals to appointments as needed.

Requirements:

Work requires the ability to read, write, and speak English and working knowledge of specific populations served, e.g., children and adolescents or adults with mental illness.
Work requires possession of a bachelor’s degree and 1 year of positively reference relevant work experience or an additional year of relevant work experience in lieu of a bachelor’s degree.
Ability to obtain additional certifications/trainings as may be required by certain programs (e.g., CPR/First Aid, Infection Control, Food Safety and Handling, and Crisis Intervention/Suicidal Protocol).
Work requires orientation period to obtain sufficient familiarity with behavioral rehabilitation methods, procedures and techniques, program policies and procedures, and specific patient population.
Must possess a valid state driver’s license. Certain programs and positions may require access to a vehicle during working hours for work related duties, including transporting clients as necessary. When access to a vehicle is required vehicle insurance must name the employee as an insured driver.
The ability to physically assist clients with daily tasks.
Good organizational, communication and time management skills.
Ability to establish rapport and to assist client to identify their needs (i.e., self-care skills, social skills, independent living skills, medication management, mental and somatic health, entitlements, and vocational) to establish appropriate rehabilitation goals and to provide internal and/or community referrals when necessary.
Working knowledge of Microsoft Office, ability to navigate through applications and utilize keyboard to input required documentation into electronic health record.
Must be able to work variety of shifts including day, evening and weekend hours and holidays.
