Job Description

The Program Coordinator, Senior Care & Caregiver Support plays a crucial role in coordinating services for older adults and caregivers so that individuals can remain living safely in the community to avoid long term care placement. You will regularly meet with clients to provide case management. We are seeking a highly motivated individual who can effectively communicate with clients, partner agencies and internal staff. The work you do will support the provision of a variety of services to eligible older adults and caregivers.

Job Requirements

• Bachelor’s degree in a related field with course work in gerontology, social work
• One or more years of related experience
• A comparable amount of education and experience may be substituted for the minimum requirement.
• Valid driver’s license
• Requires criminal background check as condition of employment.
• May be required to work outside of standard business hours.
