Job Description

Key initial contact for homeless individuals/families to navigate the process of moving from homelessness to shelter, Rapid Rehousing, Permanent Supportive Housing, and wrap-around support services.

Essential Functions:
• Lead Shelter and Housing intake process: follow established Coordinated Entry process, ensure rapid entry into services, maintain objective, equitable service delivery.
• Responsible for accurate, timely, up-to-date and complete data entry for those screened using Community Service Point, Homeless Management Information System database.
• Complete Shelter Intake Form, accurate documentation including verification of homelessness, and make specific referrals based on program eligibility.
• Maintain strong working knowledge of HSP, County, State, and Federal Housing and Urban Development (HUD) homelessness definitions and procedures; ensure the coordinated entry process and documentation of eligibility are consistent with requirements of funding agreements and fair housing laws.
• Help to oversee the Eligibility and Diversion Screening process, including entering all data from screenings and conducting an annual training to other providers in the community.
• Field inquiries about HSP’s shelter and housing opportunities for homeless individuals/families
• Track shelter/housing openings and maintain eligibility list data.
• Interview prospective participants to assess eligibility and fit with available shelter/housing and case management opportunities.
• Assess and connect participants with identified community resources such as: basic needs, mental health, substance abuse treatment, educational and employment opportunities, etc.
• Work with internal staff to facilitate smooth transition of new participants into shelter, housing, and case management.
• Understand trends, analyze data, and communicate homeless and housing needs.
• Develop and maintain a thorough knowledge of the HSP’s programs and community resources; work in tandem with HSP’s Integrated Services Team including Housing, Shelter, Opportunity WORKS (re-entry employment program), and Financial Education services to help participants access services, stabilize their lives, and secure permanent housing.
• Utilize motivational interviewing, strength-based, and trauma informed care strategies and techniques to deliver participant-driven services.
• Assemble data for HUD/Agency Annual Report, and additional monthly/quarterly reports as required.
• Participate in community partner meetings and committees as directed.

Job Requirements

Knowledge, Skills and Abilities: Candidates for this position must possess the following skills: effective interpersonal, organizational & communication skills (verbal & written) to accomplish program objectives; Comfort working directly with vulnerable populations; including people with chronic disabilities and people experiencing homelessness; Strong attention to detail with ability to manage data in both paper and electronic forms; ability to understand and work in compliance with funder requirements and regulations; Proficient computer skills (Microsoft Excel, ability to learn HMIS). Valid driver’s license with clean driving record required. Motivated and able to take initiative. Must be positive, cooperative and supportive.

Educational Requirements: Candidates for this position are required to have a high school/general diploma, associate or bachelor’s degree preferred. Experience working in shelter and housing programs preferred. Relevant experience may be considered in lieu of an associate or bachelor’s degree.

Supervisory Responsibilities: None

Work Environment: Direct interaction with vulnerable, homeless persons. Participants may have mental, physical disabilities and/or substance abuse issues. Interaction with all levels within the agency, vendors, landlords and partner agencies. Primarily office setting; will visit shelters, homeless encampments, and/or participant home setting. One evening per week is required for shelter intakes.

Position Type/Expected Hours of Work: Full Time (40 hours/week).

Travel Requirements: Minimal travel for meetings and training.
