Job Description

Responsible for assessing barriers to housing stability and providing ongoing case management to help Permanent Supportive Housing participants stabilize in housing and move on to other permanent housing opportunities. Responsible for motivating and encouraging participants to create and maintain financial stability to move out of poverty.

Essential Functions:
• Independently manage dynamic caseload, conduct monthly visits with program participants in-home and/or in-office, complete participant assessments to determine and reduce barriers to permanent housing
o Permanent Supportive Housing: provide long term case management support to chronically homeless individuals and families, promoting housing stability and Move On strategies, updating files and data entry at least once per month.
• Conduct in-home visits with program participants, using sound judgement and following all company policies and procedures to maintain safety for both participants and employees.
• Provide comprehensive financial coaching to target population (barriers include homelessness, trauma and crisis, criminal background, mental health, and substance abuse) via individual meetings or small groups.
• Facilitate monthly financial education workshops (following HSP’s established curriculum), maintain and disseminate workshop calendar, develop new materials as needed, conduct workshops via webinar, at HSP/HSP shelters, and throughout the Carroll County community.
• Maintain accurate and timely case files, per grant/funder requirements, this includes, but is not limited to appropriate participant homeless verification and demographic information, signed program agreements, referrals, service plans, and progressive case notes
• Complete accurate and timely data entry utilizing Community Service Point (CSP): complete program entry and exits within 24 hours, enter services within 3 business days, enter, track, and evaluate participant progress at identified milestones
• Develop and maintain a thorough knowledge of HSP’s programs and community resources; work in tandem with HSP’s Integrated Services Team including Housing, Opportunity WORKS (re-entry employment program), and Financial Education services to help participants access services, stabilize their lives, and secure permanent housing
• Develop and maintain a working knowledge of Housing First best practices, Housing and Urban Development Homeless Programs, Grant, and Funding goals
• Support strategies to help participant meet “Key” goals to ensure homelessness is short, brief and rare in Carroll County; work with participants to establish SMART (specific, measurable, achievable, relevant, time based) goals
• Utilize motivational interviewing, strength-based, and trauma informed care strategies and techniques to deliver participant-driven case management; develop solid rapport with participants
• Communicate with HSP’s Coordinated Entry to ensure rapid participant entry into Permanent Supportive Housing programs and attend the Coordinated Entry Co-Case Management meeting
• Assemble data for HUD/Agency Annual Report, and additional monthly/quarterly shelter reports as required

Job Requirements

Knowledge, Skills and Abilities: Candidates for this position must possess the following: eagerness to learn financial education strategies for low-moderate income individuals, effective teamwork and case management skills to accomplish the program goals and objectives; effective interpersonal and organizational skills; ability to take initiative and be self-motivating; knowledge of human service/ community resources, sensitivity to human needs. Ability to advocate on participant’s behalf. Prior experience in providing financial education services in the human services field preferred. Must possess excellent written and verbal communication skills. Must be able to maintain confidentiality. Must possess a high level of energy, cooperation, motivation, and persistence. Must be proficient in Microsoft Office (Word, Excel, Outlook, Publisher) and have accurate data entry skills. Candidates are required to have a valid motor vehicle operator’s permit and provide proof of current automobile insurance. Background check required.

Educational Requirements: Candidates for this position are required to have high school/general diploma, associate or bachelor’s degree preferred. Prior experience in the human services field preferred.

Supervisory Responsibilities: None

Work Environment: Office setting. This position has contact with all levels of personnel within the organization and participants, vendors, volunteers and community partners.

Position Type/Expected Hours of Work: Full Time (40 hours/week). Normal office hours, with some evenings and weekends.

Travel Requirements: Travel throughout Carroll County, i.e. Public Libraries, meetings, trainings and outreach events, transport participants (in company vehicle).
