Job Description

This is a direct service position requiring general and specific knowledge of mental health, community programming and entitlement programs. Primary responsibilities are to provide and/or coordinate assessment, planning, linkage, monitory and advocating for and with individuals. The In Reach Coordinator reports to the Program Manager.

Responsibilities:

Provide and/or coordinate the following services for and with assigned individuals: assessment, planning, linking, monitoring, advocacy and care both internal to Way Station’s program and through external social service agencies.
Develop working relationships with SHC patients through use of creative engagement techniques.
Meet with hospital treatment team regularly to develop and monitor transition plans.
Ensure coordinated services to individuals through effective communication with various program components and with external service providers.
Be available to address individual concerns as they arise on any given day, i.e. resource coordination, appointments, transportation, housing, skills teaching, entitlements coordination, member debt, money management, etc.
Transport, accompany and participate in individuals’ appointments as needed.
Observe and report symptoms of decompensation to appropriate staff and agencies.
Maintain regular contact with other programs, reporting any necessary information regarding staff and/or member participation in the IRP.

Job Requirements

Requirements:

Possession of a bachelor’s degree in related field or work experience that uniquely qualifies the individual for the job.
Two years of relevant work experience.
Valid driver’s license or alternative transportation plan.
