Job Description
The Guardianship Program Coordinator plays a crucial role in coordinating services to support local individuals requiring a Court-appointed guardian. You will regularly meet with clients to provide case management. We are seeking a highly motivated individual who can effectively communicate with clients, partner agencies, nursing homes, assisted living facilities and internal staff. The work you do will support the cognitive and physical well-being of clients.
Essential Duties
• Coordinate the Guardianship program and support adherence to all applicable laws and regulations.
• Using a trauma informed approach, perform case management, including determining eligibility, assessing client needs, providing information, making medical and end of life decisions for clients and securing placement for clients.
• Conduct client appointments, including field visits as required.
• Develop and maintain effective working relationships with community agencies. Provide training to community partners and citizens regarding the Guardianship Program.
• Coordinate data collection, reporting requirements, and client files. Ensure accuracy of all records maintained.
• Maintain up to date and organized case management notes and client files including court related documentation, Medical Order for Life Sustaining Treatment (MOLST) documents, burial arrangements, and other documentation.
• Demonstrate professionalism and compassion in making end-of-life decisions for clients.
• Ensure all work is complete and timely to support department goals. Evaluate feedback and procedures to improve efficiency.

• Three or more years of related experience
• A comparable amount of education and experience may be substituted for the minimum requirement.
