General Summary:
Provides service coordination for an assigned group of clients in their home or in the community, services include coordinating and monitoring the activities of the individual treatment team, assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plan in coordination with client, treatment team, symptom management, ensuring immediate changes are in the treatment plan as clients’ needs changes, educating, supporting, clients’ families and advocating for clients’ rights and preferences. This is a direct service position requiring advanced and specific knowledge of mental health and psychiatric nursing. Primary responsibility is delivery of nursing services to ACT clients with professionalism and integrity.
Responsibilities:
Provides community based mental health nursing services to individuals in their home or any place in the community that is deemed safe and appropriate.
Conducts comprehensive medical assessment and make appropriate community referral when necessary.
Participates in treatment, rehabilitation and support services.
Provides on call crisis intervention covering nighttime hours and weekends.
Participates in daily team meeting reviewing brief clinically-relevant information of all clients and contacts in the last 24 hours. Developing daily staff schedule based on weekly/monthly client schedules, emergency needs, need for proactive contacts and to prevent future crises.
Participate in treatment planning review meetings and complete individualized treatment plans
Communicate and coordinate services with other medical providers.
Engage in health promotion, prevention and education activities.
Takes lead role or participate in providing medication administration and medical services.
Under the direction of the team prescriber in collaboration with other register nurses on the team, develop , revise, maintain, and supervise team psychopharmacologic and medication treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medication prescribed by team prescriber, evaluate and chart psychotropic medication effectiveness, complication, and side effects and arrange for required lab work.
Under the direction of the team prescriber and other nurses on the team, organize, and manage the system of getting medication to clients and integrating medication administration into client’s treatment plans.
Under the direction of the team prescriber and collaboration with other registered nurses on the team manage pharmaceutical and medication supplies.
Participates in the evaluation of program effectiveness and efficiency and works to ensure continuous improvements in the quality of services.
Educate other team members to help monitor psychiatric symptoms, physical health and medication side effects in generic and specific manner.
Develop and maintain professional relationships with individuals, their support systems, other Sheppard Pratt staff and community agencies to provide coordinated and comprehensive care.
Thoroughly document all services in Electronic Health Record.
May provide general supervision to LPNs (applicable to ACT teams with RNs & LPNs on staff).
Will perform other direct job related duties as is assigned.
Job Requirements
Requirements:
Work requires knowledge on nursing theory, principles, and practice-normally acquired through graduation of an accredited program of Nursing and/or 2-3 years of direct job related experience.
Requires possession of a valid license to practice nursing in the State of Maryland. Nurses licensed as a Licensed Practical Nurse (LPN) may serve on the ACT team when at least one nurse on the team is licensed as a Registered Nurse (RN). Maintains professional knowledge and skills through attendance to professional meetings, workshops, courses, and in-service sessions.
Work requires analytical ability and sound nursing knowledge for the effective formulation of nursing care plans, implementation of care, and appropriate documentation of pertinent information regarding a patient’s condition.
Must have excellent interpersonal skills and the ability to interact effectively with psychiatrically ill patients in order to gain their cooperation, allay apprehensions and provide effective health education to patients and family members.
Must possess the ability to physically intervene in medical and behavioral emergencies, transport patients, and assist in the lifting of a patient.
Must possess vehicle during work hours, a valid state issued Driver’s License and have a current driving record with 3 or fewer points on it at the time of hire. May provide transportation to individuals in personal vehicle.
Work primarily Monday-Friday business hours, but must be flexible to adjust schedule to meet client needs including evening, weekend and on-call hours.
