General Summary:
This is a direct service position through the ACT Program. In the role of vocational specialist, this position provides services to ACT clients in all phases of vocational services including engagement, assessment, job search, job development and job support. This is an hourly position.
Responsibilities:
Work side by side with clients in their homes or other locations to complete necessary tasks including but not limited to household tasks (cleaning, cooking, outside home maintenance,) budgeting and shopping, and attendance at appointments (medical, legal, etc.).
Provide and/or coordinate the following services for and with assigned clients: assessment, planning, linking, monitoring, advocacy and care both internal to Sheppard Pratt’s programs and through external social service agencies as identified in ITP.
Meet with clients within one week of referral (identification of desire to work) and conduct initial vocational assessment
Make outreach attempts as part of initial engagement, and at least monthly when client contact
Demonstrate tolerance of different levels of readiness to work by using encouragement when a client appears unsure of working
Within first month of service, in tandem with the client, make contact with employers to secure job or to gather more information about the work site
Assist clients in finding jobs that are interesting and meaningful to them and at the skill level they desire by making employer contacts that are based on the client’s job
Conduction individualized job development by talking to employers about specific client’s abilities and desires to do a job in their place of business and using extensive networking to identify potential job leads. (Talking with family members, friends, staff members, previous employers, community members)
Provide individualized, time-unlimited follow along supports to clients including crisis intervention, job coaching, counseling and job support groups to help maintain employment
Provide individualized time-unlimited follow along supports to employers
Communicate regularly with team members regarding vocational issues
Promote employment options in team meetings, ITP reviews and other activities even when other staff members may not identify them as candidate for work
Share success regularly with members of the team
Provide education and consultation to team members by sharing knowledge of the client in relationship to work
Assist in facilitation of client activity as assigned.
Facilitate the development and implementation of individual plans of assigned clients to meet the client’s needs, interests and goals and coordinate communication and activity amongst staff working together with assigned clients.
Observe and report any changes in individuals’ conditions, physical and/or mental health.
Manage difficult or emotional individual situations, respond professionally for requests for assistance and support and meets commitments to individuals and to team.
Provide transportation to individuals in personal vehicle
Provide transportation in company owned vehicles including large passenger vehicles as assigned
Assess for health and safety and take action with individuals, groups and environment.
Monitor medication as assigned.
Respond to crisis situations
Support individuals with money management and budgeting as assigned
Serve as an advocate for individuals served
Develop and maintain professional relationships with individuals, their support system, other Sheppard Pratt staff and community agencies to provide coordinated and comprehensive care
Plan coverage for absences in conjunction with your supervisor
Participate in 24/7 after hours on call system for ACT clients
Attend daily team meeting to review client statuses and needs
Complete training and documentation as required by regulation and accreditation bodies
Thoroughly document all services
Assist in monitoring other staff, students and volunteers as assigned
Assist with maintenance of program space, equipment and supplies
Participate in improving program effectiveness and efficiency by contributing to program evaluation, as well as working to ensure continuous improvements in the quality of services
Perform other duties as assigned
Job Requirements
Requirements:
Education: Bachelor’s degree in related field
Professional Experience: Two years of positively referenced work, student or volunteer
experience
Computer Skills: Working knowledge of Microsoft Office, ability to navigate through applications and utilize keyboard to input required documentation into electronic health record
Driving Record: Safe Driving Record
Background Check: Satisfactory Criminal Background check
Transportation: Possession of a 4 passenger vehicle that meets company inspection, valid Driver’s license and vehicle insurance that names employee as insured driver
Availability of work schedule: Work primarily M-F business hours but must be flexible to adjust schedule to meet client needs including evening or weekend hours
